At Constellation, we are proud to support & recognize veterans. 

Our compassionate professionals are equipped to support veterans emotionally, spiritually, and physically. It is our duty and privilege to serve the heroes that served our country. For that reason, we offer a four-step process to recognize veterans as part of our hospice services. 

Recognize

 Starting at admission, we work with veterans and their families to understand their unique stories and personalize a plan. 

Thank

 Our team offers thanks and appreciation to veterans upon every visit, because we believe affirmation matters. 

Acknowledge

 Constellation presents a gift of gratitude to veterans during their time in hospice. Family, friends and caregivers are invited. 

Honor

 We honor individuals the entire time they are in our care including bereavement communications where our appreciation for them continues.
