Description
A patient with Chronic Obstructive Pulmonary Disease (COPD) was found in respiratory distress during two home health visits. In addition to COPD, the patient had also been diagnosed with heart and liver disease. To mitigate an escalating episode, Constellation added a respiratory therapist (RT) to the patient’s care team.
Complete Care Approach
The RT worked to get the patient’s breathing under control by utilizing certain medications and breathing techniques. A telemonitor was brought into the home to give real-time insights into the patient’s oxygen saturation levels. Through advanced clinical oversight by a tightly connected RT and pulmonologist on staff, the patient remained “tucked in” safely at home.
Remote Patient Monitoring: Telemonitoring allowed clinicians to monitor patient in real-time.
Advanced Care Team: Highly coordinated care by the RT and pulmonologist.
Delivery Service: Medication order prevented an in-person office visit.
