Description
During the height of the COVID-19 pandemic, a patient living in an assisted living facility (ALF) was admitted to the hospital with unresponsiveness, shortness of breath, anxiety, and rigidity.
Complete Care Approach
Constellation worked to have all medical necessities ordered and delivered to the assisted living facility (ALF) before the patient returned from the hospital. Due to COVID-19, visitors were not allowed in the facility. To coordinate a visit with family, the nurse provided the patient with an iPad to virtually connect with family and a bilingual chaplain’s support from outside the window. Working with the ALF staff, Constellation educated the family about end-of-life while ensuring the individual was comfortable. The nurse held the patient’s hand and coordinated visitation while the patient passed away. Bereavement support was offered to family and staff at the ALF. The family wrote Constellation a thank you letter for safely coordinating visitation during their loved one’s passing.
Delivery Service: New medical supplies and medication delivery were established to accommodate the patient’s needs.
Virtual Visits: Connecting individuals to family members and loved ones via iPad.
