Constellation home care offers behavioral health services for patients who are diagnosed with a psychiatric condition.  We use a comprehensive approach to provide education in the areas of physical, cognitive, affective, and social functioning.  Our team will work with the patient’s family, physician and interdisciplinary care team to personalize and adjust a care plan throughout the recovery process.

The goal of our behavioral health professionals is to empower our patients to reach their highest level of independence by educating the individual, the individual’s care team and the individual’s family.

A few Examples of diagnoses our team has managed:  Mood Disorders, Psychotic Disorders, Dementia, Anxiety Disorders, Personality Disorders, Substance Abuse, Chronic Mental Illness.

Constellation’s team of Behavioral Health Supervisors and Registered Nurses all have experience in crisis intervention, community assessments, and inpatient/outpatient settings.  They regularly attend trainings to stay up to date in both methods of care and community resources.


Areas of assessment and education can include:

  • Our Team of Trained Behavioral Health Specialists Include:  Registered Nurses, Occupational Therapists, Physical Therapists, Social Workers, and Home Health Aides.
  • Personal Care:  Dressing, Grooming, Bathing and personal Hygiene.
  • Meal Preparation:  Creating a shopping list, purchasing groceries, cooking, storing food.
  • Self-Cueing Techniques:  Medication management tools, scheduling meal times, creating a cleaning schedule and upkeep of environment.
  • Safety in Home:  Instructions and education on how to safely use appliances, washer and dryer, microwave oven and correct safety procedures within the patient’s surroundings.
  • Financial Management:  how to create and manage a budget.
